SHIPPING INSTRUCTIONS
Before SHIPPING your product for service, please fill out the following form. Before you submit the form print a copy and place it in the box with your product. If Possible please send unit in original packaging. If not original box, Double box the unit when shipping your product, package it carefully using at least 3" of packing material on all sides to avoid damage. Insure the package for the value  of the product. If unit not packaged properly, customer will be charged for packing material. Also, if unit not packaged properly and gets damage Warranty will not cover repair.  Include a copy of Proof of Purchase and original remote.

All equipment mailed in must be packaged in either the original packing or double box unit. Most packing materials such as styrofoam can get into your equipment through small holes in the chassis and cause problems in the mechanism. Please wrap your equipment in a plastic bag before adding packing material to the box.
